Short-Term Disability Insurance:
Employees also have the option of purchasing short-term disability insurance to protect themselves from loss of income during the initial six months of disability, pending the start of long-term disability or social security disability, if applicable.

Long-Term Disability Insurance (LTD):
If you should become disabled for more than six months, LTD payments at the rate of 60 percent of pay (including other sources of income) are provided. All regular full-time and part-time (budgeted to work 32 hours or more) employees who have completed one year of employment are eligible for this benefit that is provided by Univeristy Health System free of charge.

Group Term Life Insurance:
All regular full-time and part-time (budgeted to work 24 hours or more) employees are covered under group term life insurance. A term insurance policy in the amount of $4,000 is paid for by Univeristy Health System, and an employee may purchase optional insurance at the group rate in an amount equal to a total of one or two times his or her annual salary. Accidental death and dismemberment coverage is included free of charge.

Dependent Group Life Insurance:
Employees eligible for life insurance can purchase $10,000 of coverage for their spouse and $5,000 of coverage for each child.

Universal Life Insurance:
In addition to the group term life insurance, employees (budgeted to work 20 hours or more) have the option of purchasing group universal life insurance for themselves and their families. This insurance can be retained when the employee leaves University Health System, with no increase in premium. Universal life insurance also builds cash value.

Cancer, Dread Disease, Intensive Care Unit Coverage:
Employees have the option of purchasing coverage for services that are typically not covered by your medical insurance policy, such as transportation to specialty treatment centers, private duty nursing, travel and lodging, as well as loss of income.

Flexible Spending Accounts:
All employees are eligible to establish pre-tax spending accounts to pay for dependent care and health care costs with pre-tax dollars. Participation is completely voluntary.

Choice CARE
Employee Assistance Program
Choice CARE is a free Employee Assistance Program (EAP) provided by UHS. This benefit provides confidential counseling, information and assistance to solve personal problems of any type. Professional counselors are available 24 hours a day, seven days a week. Marriage, family alcohol/drug, debt, legal and emotional are just some of the problems Choice CARE can help with.
