Application Requirements

  1. Letter of interest/intent
  2. Current curriculum vitae
  3. Official copies of all college transcripts, including pharmacy school
  4. Three professional letters of recommendation
    • ASHP forms will be accepted, but actual letters are also required
  5. University Health System Job Application
  6. Recent photograph (color or black & white)

With the exception of the UHS Job Application, the application materials listed above must also be submitted to The University of Texas at Austin College of Pharmacy Residency Program, along with the Residency Application for The University of Texas (http://www.utexas.edu/pharmacy/divisions/pharmaco/postdoc), to:

Once accepted into the residency program, candidates must obtain a Registered Pharmacist License in the State Of Texas by August 1.
